📋 Description

• Contact members and community partners to support healthcare initiatives and activities • Conduct educational and quality campaigns • Assist with scheduling care manager and member appointments • Perform ongoing functions related to members' health-related social needs directly or through social service departments and community-based organizations • Respond to member inquiries • Support clinicians in physical and behavioral health care management with provider and member activities • Support relationships with community partners and work with community-based organizations • Contribute to the design of community health outreach programs • Perform field-based work as required by initiatives, health campaigns, and departmental functions • Manage low-risk members for health-related social needs, education, basic outreach screenings, assessments, and inquiries

🎯 Requirements

• Bachelor's degree OR 1 year of college and 3 years of health, social service or community experience OR 4 years' health, social services or community experience • Knowledge of medical terminology • Experience coordinating member-related needs, providing assistance to members, and analyzing member needs • PC proficiency including Microsoft Office applications • Customer service skills • Verbal and written communication skills, including developing written correspondence to members and department personnel • Coaching skills, including motivational interviewing, to educate members on medical and behavioral health issues • New Mexico only: Bachelor's degree in Social Work • New Mexico only: Proficient in a New Mexico Native American Pueblo language; must be able to translate and present • New Mexico only: Community Health Worker (CHW) certification • Preferred: Community engagement • Preferred: Supporting populations with health-related social needs • Candidate must reside in the Las Cruces, New Mexico area • Remote work permitted within the continental United States, excluding California, New York, Alaska, and Hawaii
