📋 Description

• Run intake, triage, and case management for reported safety events • Maintain response timeframes and case status discipline through closure • Build a reporting culture that surfaces safety events • Conduct reviews of serious events • Code contributing factors, harm severity, and care attribution to a reference taxonomy • Escalate to and collaborate with clinical leadership when necessary • Convert findings into corrective and preventive actions with named owners and due dates • Track corrective actions to verified closure • Extract learning from events and trends and translate it into system improvements • Use data to drive risk analysis and mitigation efforts • Maintain the reference taxonomy and crosswalks between reporting pathways • Analyze trends in reported events and contributing factors • Produce metrics, trend analysis, and case narratives for quarterly Quality and Patient Safety Reports • Support the Patient Safety Review Committee • Draft and maintain safety policies and event management protocols • Run debriefs and lessons-learned communications • Champion a just culture approach • Structure internal investigations to meet regulatory, accreditation, and delegated credentialing standards • Partner with Legal and Compliance when needed • Collaborate with clinical operations, legal, compliance, product, and data teams • Study and amplify the culture of safety

🎯 Requirements

• 5+ years experience in healthcare risk management or patient safety • Experience in behavioral health settings, whether outpatient, telehealth, crisis or inpatient • Direct experience owning event investigation and corrective action • Experience working from established method and standard work at high case volume • Sound triage and escalation judgment • Ability to distinguish clinical questions from operational or legal questions and route appropriately • Fluency with data, including defining denominators, critically reading dashboards, and developing plans for deeper analysis • Ability to drive corrective actions to verified closure across teams • Ability to build reporting culture through relationships and just culture • Ability to present findings to senior clinical and business leaders • Deep commitment to ethical, patient-centered care • Track record of developing high-impact and high-volume safety programs • Strongly preferred: CPHRM, CPPS, or equivalent patient safety credential • Actively licensed clinician (RN, LCSW, LPC, licensed psychologist, or equivalent clinical credential) preferred but not required • Accreditation or delegated credentialing survey experience preferred but not required • Authorization to work in the United States for any employer
