📋 Description

• Own the operating system for reported safety events from intake and triage through investigation, systematic event analysis, corrective action, and case closure • Build a reporting culture that surfaces safety events • Lead investigations of serious events, including root cause and apparent cause analyses • Define investigation standards and the reference taxonomy used at scale • Convert findings into corrective and preventive actions with named owners and due dates • Track corrective actions to verified closure across teams • Translate event trends into upstream fixes that prevent future harm • Respond first to high-risk clinical events and provide timely, compassionate, evidence-informed support • Draft and maintain patient safety and event management protocols and policies with Clinical Operations, Legal, and Compliance • Define safety metrics and baseline event rates • Own the Clinical Risk Dashboard and analyze reported-event trends and contributing factors • Produce metrics, trend analysis, and case narratives for quarterly Quality and Patient Safety Reports • Support the Patient Safety Review Committee and peer-led governance committees • Structure investigations to meet regulatory, accreditation, and delegated credentialing expectations • Supply evidence of clinical oversight for payer and accreditation reviews • Build and execute project plans for new patient safety programs • Partner with clinical product, clinical operations, legal, compliance, and data teams • Help shape staffing and work division as the function grows

🎯 Requirements

• Active clinical license as a master’s level therapist (LCSW, LPC, LMFT), doctorate clinical psychologist (PhD/PsyD), or psychiatric clinician (PMHNP-BC, MD/DO) • 7+ years of experience in healthcare patient safety or clinical risk management, including direct ownership of event investigation and corrective action • Built or substantially scaled a safety event or clinical risk program • Experience developing and managing a program from project plan through launch and scale • Fluency with data, including defining metrics, critically reading dashboards, interpreting trends, and planning deeper analysis • Ability to drive corrective actions to verified closure across teams • Exceptional written and verbal communication skills • Deep commitment to ethical, patient-centered care and continuous quality improvement • Adaptability with new technology and comfort being the first person in a growing role • Certification in Quality and Patient Safety, Healthcare Quality and Management, Healthcare Risk Management, or an equivalent patient safety credential — strongly preferred • Authorization to work in the United States for any employer • Must disclose whether visa sponsorship is required now or in the future
