• Receive incoming calls from members regarding benefit questions/issues, care coordination, program inquiries, connections to care managers, and screening assessments • Document calls contemporaneously and accurately in the designated PHI system • Manage an assigned caseload of members requiring short-term care coordination and health navigation • Work within the interdisciplinary care team to communicate member issues or needs promptly • Monitor member screening effectively to improve quality and cost outcomes • Perform outreach calls to confirm services are in place • Perform additional duties as assigned
• Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), previous relevant medical training, or relevant work experience • High school diploma/GED • Work experience requiring effective verbal and written communication • Good grammar, spelling, and punctuation skills • Experience working in a fast-paced environment while handling multiple priorities simultaneously • Preferred: Associate’s Degree • Preferred: prior experience in a call center, care management, or healthcare environment • Preferred: familiarity with medical terminology, Medicaid, and Child Health Plus benefit products • Excellent telephone, organizational, and customer service skills • Organized and detail-oriented team player
