📋 Description

• Conduct virtual consultations with patients, focusing on education about lifestyle and behavior related to clinical status • Manage an assigned patient load based on practice need • Review and respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, following approved partner workflows • Document patient communication in patient records • Assist patients with setting up their devices • Communicate with enrolled patients in conjunction with their care teams • Establish relationships with assigned patients to empower chronic illness self-management and improve health • Collaborate with other team members when assistance is needed • Perform additional duties as assigned

🎯 Requirements

• Current, valid, and in-good-standing New York Nursing License (LPN/LVN) required • Current, valid, and in-good-standing Multistate/Compact Nursing License preferred • 3+ years of experience in cardiology practice preferred, or experience in primary care practice, home care, or chronic care management • Experience in remote monitoring and/or virtual care management programs preferred • High level of computer literacy and comfort learning and using a digital health portal • Ability to work independently • Excellent time management and organizational skills • Engaging personality and desire to learn new topics in lifestyle medicine • Ability to acquire, interpret, and convey clinical research • Attention to detail • HIPAA-compliant remote work area • Distraction-free work environment • Reliable internet with minimum 100 Mbps download and 10 Mbps upload • New York residency/license eligibility indicated by the role’s New York-specific requirement
