📋 Description

• Assess and evaluate members' needs and requirements in a telephonic environment • Guide members and families toward appropriate care and wellbeing resources • Manage members' physical, environmental, and psychosocial health issues • Identify and resolve barriers to effective care • Monitor patient care through assessments and evaluations to ensure progress toward desired outcomes • Create member care plans as needed • Perform health assessments over the phone • Provide care planning and education on chronic conditions and gaps in healthcare plans • Make independent decisions regarding work methods while following established guidelines and procedures • Work with an assigned member population

🎯 Requirements

• Bi-Lingual (English/Spanish) • Bachelor's degree • 3 - 5 years of clinical acute care experience • Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook • Passion for contributing to an organization focused on continuously improving consumer experiences • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information • Self-provided internet service with at least 25 Mbps download speed and 10 Mbps upload speed • Availability for working hours between 8:00am-7:00pm Central Standard Time, with core hours primarily 8:00am-5:00pm CST • Occasional travel to Humana offices for training or meetings may be required
