📋 Description

• Assess pregnant and postpartum Medicaid members' care needs • Ensure members receive appropriate services in the least restrictive setting • Identify and resolve barriers to perinatal and postpartum care • Plan and implement interventions to meet care needs • Coordinate services and monitor and evaluate case management plans against members' personal goals • Continuously monitor member progress using assessments, data, conversations, and active care planning • Conduct telephonic and face-to-face assessments and evaluations • Remediate immediate and acute gaps in care and access • Explain MCO requirements, services, and benefits to enrollees • Provide referrals to community resources • Guide members and families toward appropriate care resources • Facilitate interactions with payers, providers, interdisciplinary teams, and others involved in care • Visit Medicaid members in their residences • Travel locally up to 25% for collaboration, face-to-face meetings, and field interactions • Work with nurses, social workers, behavioral health professionals, and care coordinators to improve health outcomes and address barriers to care

🎯 Requirements

• Illinois Medicaid-Certified Doula • Must reside in Illinois • 1+ years of experience in health care • 1+ years of case management/care coordination experience • Intermediate to advanced computer skills • Experience with Microsoft Word, Excel, and Outlook • Experience using electronic information applications/software programs, including electronic medical records • Valid state driver's license • Personal vehicle liability insurance meeting state minimum limits or specified Humana limits • Must complete TB screening if selected • Must maintain internet service with at least 25 Mbps download and 10 Mbps upload speeds • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information • Ability to work primarily Monday–Friday, 8:00 AM–5:00 PM CST, with flexibility based on business needs
