📋 Description

• Assess and evaluate members' needs and requirements by telephone and face-to-face • Guide members and families toward appropriate care and wellbeing resources • Monitor care, assessments, and evaluations to ensure members progress toward desired outcomes • Create member care plans • Manage behavioral, physical, environmental, and psychosocial health needs using varied strategies and techniques • Identify and resolve barriers to effective care • Collaborate with providers and community services to promote quality and cost-effective outcomes • Coordinate in-person meetings when face-to-face enrollee support is required • Work collaboratively with an interdisciplinary care management team including community health workers, housing support specialists, SDOH coordinators, and care management support assistants • Follow established guidelines and procedures

🎯 Requirements

• Must reside in Oklahoma • Active Registered Nurse (RN) license, or a Licensed Master Level Social Worker (LCSW, LPC, LMFT) • Minimum 1-year Behavioral Health clinical experience • 2 or more years of experience in in-home case or care management • Experience working with Medicaid and/or Medicare enrollees to coordinate services, care needs, or benefits • Knowledge of community health and social service agencies and additional community resources • Comprehensive knowledge of Microsoft Office applications, including Word, Excel and PowerPoint • Periodic travel to Humana Oklahoma City office for meetings and training • Case Management Certification (CCM) preferred • Valid driver's license with reliable transportation and ability to travel within the state • Proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits • Ability to meet Humana's home internet requirements: minimum 25 Mbps download and 10 Mbps upload • Ability to work from a dedicated space without ongoing interruptions to protect PHI/HIPAA information • Must complete tuberculosis (TB) screening if selected
