📋 Description

• Assess and evaluate members' needs and requirements in a telephonic environment • Guide members and families toward appropriate care and wellbeing resources • Manage members' physical, environmental and psycho-social health issues using varied strategies and techniques • Identify and resolve barriers to effective care • Continuously monitor patient care through assessments, data, conversations with members, and active care planning • Navigate multiple systems and use critical thinking skills • Perform health assessments, care planning, and chronic-condition education over the phone • Identify gaps in members' healthcare plans • Work with an assigned member population • Make independent decisions regarding work methods and appropriate courses of action while following established guidelines and procedures

🎯 Requirements

• Bachelor's degree • 3+ years of clinical acute care experience • Licensed Registered Nurse (RN) in the state of Illinois with no disciplinary action • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook • Passion for contributing to an organization focused on continuously improving consumer experiences • Experience with case management, discharge planning and patient education for adult acute care • Managed care experience • Certified Case Manager (CCM) • Bi-Lingual (English/Spanish) • Internet service with at least 25 Mbps download speed and 10 Mbps upload speed • Dedicated work space without ongoing interruptions to protect member PHI / HIPAA information
