📋 Description

• Provide comprehensive, holistic case management for the Special Populations program throughout the continuum of care • Deliver specialized case management interventions using appropriate care sites and levels of care • Prevent or decrease hospital readmissions • Promote appropriate utilization of alternative funding sources • Enhance coordination of care and reduce fragmentation of clinical services • Serve as the Special Populations Program beneficiary's point of contact for clinical-spectrum activities • Coordinate and collaborate with Care Management programs, Military Treatment Facility staff, physicians, and providers • Ensure fully integrated care plans address all beneficiary needs and conditions • Report to a Manager of Care Management

🎯 Requirements

• U.S. citizenship required • Successfully receive interim approval for government security clearance (NBIS - National Background Investigation Services) • Candidates currently living in Puerto Rico cannot be hired • Active RN license in state of employment • Ability to obtain CCM certification within 1 year of hire • 2 years case management experience • 3 years clinical RN or managed care RN experience • Ability to work an 8-hour shift Monday–Friday between 8:00 a.m. and 6:00 p.m. EST • Mandatory training for the first 4 weeks from 8:00 a.m. to 5:00 p.m. EST • Minimum internet speed of 25 Mbps download and 10 Mbps upload • Dedicated, interruption-free workspace to protect member PHI/HIPAA information • Preferred: Bachelor's degree in nursing • Preferred: TRICARE experience • Preferred: Experience with MCG • Preferred: Current CCM
