📋 Description

• Provide comprehensive case management services to frail elders and adults with intellectual, developmental, or physical disabilities in the Wisconsin Family Care program • Assess members' health and safety needs and develop comprehensive Member Care Plans • Coordinate with a Field Care Nurse to address members' health and safety needs in the least restrictive environment • Conduct face-to-face social assessments upon enrollment and at least every six months, typically at members' residences • Conduct quarterly in-person home visits and maintain monthly phone contact • Arrange support services related to social integration, community resources, employment, housing, and other non-medical needs • Ensure cost-effective service delivery • Evaluate risk factors and educate members • Maintain accurate case notes, service authorizations, and Member Care Plan updates • Serve members in Green and Lafayette counties • Report to the Manager of Care Coaching

🎯 Requirements

• Bachelor's degree in human services or a related field, with at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a Bachelor's degree in another field with at least 3 years of such experience • Travel up to 40% of the time to visit members • Valid state driver's license • Proof of personal vehicle liability insurance with at least 100/300/100 limits • Reside within 45 minutes or less of the assigned coverage area (preferred) • Case Management experience (preferred) • Experience with electronic case note documentation (preferred) • Knowledge of community health and social service agencies and additional community resources (preferred) • Patient-facing role requiring TB screening • Minimum home internet speed of 25 Mbps download and 10 Mbps upload for home or hybrid home/office employees • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
