📋 Description

• Provide comprehensive case management services to frail elders and adults with intellectual, developmental, or physical disabilities in the Wisconsin Family Care program • Support members in Dane/Columbia and surrounding areas • Assess members' health and safety needs, strengths, interests, and preferences to develop a comprehensive Member Care Plan (MCP) • Coordinate with a Field Care Nurse to address members' health and safety needs and provide support in the least restrictive environment • Conduct face-to-face social assessments upon enrollment and at least every six months, typically at members' residences • Conduct quarterly in-person visits and maintain monthly phone contact • Arrange support services related to social integration, community resources, employment, housing, and other non-medical needs • Ensure cost-effective service delivery • Evaluate risk factors and educate members • Maintain accurate case notes, service authorizations, and MCP updates • Travel up to 40% across Dane and Columbia counties and surrounding areas • Report to the Manager of Care Coaching

🎯 Requirements

• Bachelor's degree in human services or a related field, with at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities • Alternatively, a Bachelor's degree in another field with at least 3 years of such experience • Valid state driver's license • Proof of personal vehicle liability insurance with at least 100/300/100 limits • Preferred: Reside within 45 mins or less of the assigned coverage area • Preferred: Case Management experience • Preferred: Experience with electronic case note documentation • Preferred: Knowledge of community health and social service agencies and additional community resources • Patient-facing role requiring TB screening if selected • Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information • Self-provided internet service with at least 25 Mbps download and 10 Mbps upload speeds
