📋 Description

• Administer ongoing long-term services and support (LTSS) assessments using person-centered thinking approaches • Contact members telephonically or in person to establish goals and priorities • Evaluate resources, develop plans of care, and identify LTSS providers and community partnerships • Develop and modify service plans and involve applicable care-team members in care planning • Support members with navigation of LTSS and environmental and social needs • Help members and caregivers access long-term services, social, housing, educational, and other services • Build trust and promote independence through collaborative relationships with Care Coordinators, members, and caregivers • Identify transition opportunities and work with transition coordinators to support member choice • Coordinate referrals for non-capitated services and capture all member services and natural supports • Coordinate and consult with Humana-contracted providers regarding LTSS delivery • Participate in interdisciplinary care team meetings • Connect and refer members to community resources and third-party payers • Assist members in maintaining Medicaid eligibility • Collaborate with Medical Directors, Geriatricians, and Care Coordinators to ensure cohesive service delivery and positive member outcomes • Meet members in their locations and spend time assessing needs and barriers

🎯 Requirements

• Bachelor's degree in health and human services field • 2 or more years of related experience • Proficiency with a variety of electronic information applications/software programs • Must participate in Humana's tuberculosis (TB) screening program • Passion for contributing to an organization focused on improving consumer experiences • Valid state driver's license • Personal vehicle liability insurance meeting the residing state's minimum limits or Humana's specified limits, whichever is higher • Separate room with a locked door usable as a private home office • Must reside in Blackford, Delaware, Grant, Henry, Jay, Madison, or Randolph County, Indiana • Prior experience with Medicare and Medicaid recipients preferred • Experience with electronic case note documentation and multiple computer applications preferred • Experience with health promotion, coaching and wellness preferred • Knowledge of community health and social service agencies and community resources preferred • Home internet with at least 25 Mbps download and 10 Mbps upload speeds • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
