📋 Description

• Serve as a Float Case Manager for members in Wisconsin's Family Care program • Cover caseloads for team members in the SW Pod 2 region who have exited their role or are on leave • Assess members' health and safety needs, strengths, interests, and preferences • Develop comprehensive Member Care Plans (MCPs) • Coordinate with a Field Care Nurse to provide services aligned with each MCP • Conduct face-to-face social assessments at enrollment and at least every six months • Conduct quarterly in-person visits and maintain monthly phone contact • Arrange support services related to social integration, community resources, employment, housing, and other non-medical needs • Ensure cost-effective service delivery • Evaluate risk factors and educate members • Maintain case notes, service authorizations, and MCP updates • Travel to members' settings approximately 40% of the time • Travel throughout the southwest Wisconsin region, with occasional travel to Humana offices for training or meetings

🎯 Requirements

• Four-year bachelor's degree in Human Services or a related field and one (1) year of experience working with at least one Family Care target population, OR a four-year bachelor's degree in any other field and three (3) years' related experience working with at least one Family Care target population • 3 or more years of Family Care experience • Must reside in or near the southwest region of Wisconsin • Valid state driver's license • Proof of personal vehicle liability insurance with at least 100/300/100 limits • Must complete tuberculosis (TB) screening if selected • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
