📋 Description

• Mentor and support Field Nurses and Care Coaches/IDT to ensure Family Care benefits comply with the DHS contract • Identify learning needs and arrange mentorship, orientation, and ongoing learning for IDT staff • Assess and coach staff on IDT competencies and performance standards • Review reports, assess processes, and review documentation • Assess utilization and quality of care management for members served • Collaborate across departments to ensure quality and effectiveness of provider services • Educate community partners • Participate in hiring by selecting, orienting, and training associates • Facilitate in-person unit meetings and attend quality visits • Provide daily leadership and mentoring to staff • Report to the Associate Director, Care Coaching • Supervise approximately 11–15 direct reports • Travel occasionally to Humana offices for training or meetings

🎯 Requirements

• Bachelor's degree in human services or a related field or associate degree in nursing • 1 year of leadership experience in the human services field • 3 years of experience working in the Family Care program • Preferred: 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities or frail elders • Preferred: Reside within 2 hours of Dane County • Preferred: Master's degree in public administration, social work or related field • Ability to work Monday–Friday, 8:00 am–4:30 pm CST • Ability to travel at least once per month, with potential for more travel • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
