📋 Description

• Lead teams of nurses, social workers, behavioral health professionals, and care coordinators managing Medicaid members in Humana's Illinois market • Oversee assessment and evaluation of members' needs to support optimal wellness • Support and enhance care management models using community health workers, peer support specialists, housing advocates, and other extenders • Promote culturally responsive, trauma-informed, and person-centered care management • Foster partnerships with community-based organizations, aging services, housing providers, and public agencies • Monitor program performance, analyze data, identify gaps, and drive continuous improvement • Lead a multidisciplinary team of Care Managers, Social Workers, and member support associates • Align departmental processes and performance with market and enterprise objectives to control costs and improve operational efficiency • Collaborate with internal teams and external partners to integrate non-clinical support associates into care planning and service delivery • Ensure compliance with federal, state, and contractual requirements • Develop policies and procedures supporting consistent, high-quality service delivery, health equity, and disparity reduction • Create a productive department through communication, briefings, team meetings, and collaboration with Care Management leadership • Support training and capacity-building for care management teams • Assist with member issues involving housing, food insecurity, transportation, and other social needs • Represent the care management program in initiatives, advisory groups, and community forums • Participate in management activities supporting Humana's strategic growth and development • Coordinate operational support through efficient workflows and quality product delivery • Improve customer satisfaction through program monitoring and timely, appropriate service delivery • Report to the Associate Director, Care Management • Direct up to 15 associates • Travel up to 50% for field-based oversight visits, member/family meetings, community partner meetings, and care team interactions • Attend occasional onsite meetings at Humana's Illinois locations

🎯 Requirements

• Must reside in Illinois or a border state • Bachelor's degree • Active Registered Nurse (RN) license or Social Work license (e.g., LCPC or LCSW) • 5+ years of professional experience • 2+ years of management or supervisory experience • Knowledge of IL Medicaid guidelines, benefits and policies and procedures • Strong understanding of care management models and the role of extenders in addressing social needs • Proficiency in analyzing and interpreting data trends • Progressive operational leadership experience • Demonstrated ability to lead cross-functional initiatives and collaborate with external partners • Ability to operate independently and in a team environment • Valid state driver's license • Personal vehicle liability insurance meeting required limits • Required TB screening upon selection • Ability to work Monday-Friday, 8:00 AM-5:00 PM CST, with flexibility based on business needs • Applicants speaking with members in a language other than English must complete the Interagency Language Rating (ILR) language proficiency assessment • Home internet with minimum 25 Mbps download and 10 Mbps upload speeds • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
