📋 Description

• Assess members' health and safety needs to develop comprehensive Member Care Plans • Conduct regular health assessments • Coordinate with acute and primary care providers • Educate members on disease processes, related risks, nutrition, exercise, and lifestyle modifications • Document in member files within required DHS contract timeframes • Participate in monthly member phone contacts • Travel to attend in-person visits in members' settings • Complete record reviews for reassigned members and identify needed record remediation • Support the team with record review and remediation • Complete remediation documentation • Inform newly assigned RNs and Care Coach partners of missing or needed follow-up • Assign tasks in the D365 system • Accept additional complex member assignments • Provide support with new member assignments • Report to the Manager of Care Coaching • Support members enrolled in the Wisconsin Family Care Program through care coordination, support, and education

🎯 Requirements

• Associate degree in nursing • Licensed Registered Nurse in the state of Wisconsin, in good standing • 3 years Care Management experience preferred • Reside within 45 mins or less of the assigned coverage area preferred • Previous Care Management experience preferred • Valid state driver's license • Proof of personal vehicle liability insurance with at least 100/300/100 limits • Required to undergo TB screening if selected • Home internet with minimum download speed of 25 Mbps and upload speed of 10 Mbps • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Ability to work Monday through Friday, 8:00 am–4:30 pm CST • Willingness and ability to travel approximately 40% of the time
