📋 Description

• Support population health initiatives by optimizing medication use and improving patient outcomes • Collaborate with pharmacists and healthcare professionals to manage medications, improve adherence, and address medication-related issues • Monitor medication adherence, identify gaps in care, and support value-based care interventions • Assist with medication reconciliation and maintain accurate medication lists during transitions of care • Contact patients, provide medication education, and address barriers to adherence • Collect and analyze medication-use, adherence, and outcomes data • Coordinate care with pharmacists, physicians, nurses, and other healthcare professionals • Educate patients and families about medications, side effects, and adherence • Contribute to quality, cost-reduction, and performance-metric initiatives • Generate reports on medication adherence, therapeutic outcomes, and related metrics

🎯 Requirements

• Reside in Colorado • Active Pharmacy Technician license in Colorado • Completion of a pharmacy technician training program or at least two years of experience as a pharmacy technician • Current pharmacy technician certification and/or license in state of practice • Experience in Medicare Stars, medication adherence, and quality measures for top candidates • Experience in value-based care or medication management settings preferred • Outpatient or retail experience preferred • Ability to work Monday through Friday, with no weekends or holidays • Ability to work 8:00 am to 4:30 pm MST • Ability to complete an on-camera recorded video interview through Microsoft Teams • Ability to attend onsite interviews and/or meetings • Ability to see and read information and labels, assess patient needs, operate monitors, and identify equipment and supplies • Ability to verbally communicate and hear and understand spoken information, alarms, needs, and issues quickly and accurately • Manual dexterity to manipulate complex and delicate equipment with precision and accuracy • Frequent computer use and typing for documenting patient care, accessing information, and medication preparation
