📋 Description

• Identify patients for proactive interventions using screening criteria, medical record review, payor models, medical risk scores, or referrals • Assess patients' medical, functional, and social conditions to develop individualized care plans, recommendations, or referrals • Coordinate internal and external services for social determinants of health needs and community care • Evaluate and modify care plans and interventions based on effectiveness and outcomes • Conduct remote nursing assessments using critical thinking and evidence-based protocols to determine care recommendations • Support patient self-management and behavior change through health coaching, care navigation, care coordination, and education • Educate healthcare team members about transitions, triage processes, and referrals; educate patients about conditions and self-management • Collaborate with physicians and healthcare team members to ensure timely, quality care and resolve delays or issues • Participate in rounds or case conferences and consult team-based resources such as social work, nutrition, pharmacy, rehabilitation, and behavioral health • Build partnerships with hospital care management, post-acute providers, and other care managers to ensure seamless transitions and continuity of care • Participate in system and regional initiatives to improve transitions of care and avoid duplicative services • Conduct root cause analysis of extended post-acute stays, inappropriate utilization, and readmissions; track key metrics • Monitor industry, regulatory, technology, and market trends affecting ambulatory and post-acute services • Promote Intermountain Health's mission, vision, values, and service behavior standards

🎯 Requirements

• Current Registered Nurse (RN) license in state of practice • Bachelor of Science in Nursing (BSN) from an accredited institution; degree verification required • RNs hired or promoted into this role must obtain their BSN within four years of hire or promotion • Demonstrated clinical nursing experience in chronic disease management • Familiarity with chronic disease terminology and processes • Demonstrated understanding of disease management, including treatment, length of stay, barriers to care delivery, and variation • Basic computer skills and knowledge of Microsoft Office software • Care Management Certification preferred • Experience in ambulatory transitional care or telephonic triage preferred • Intermediate computer skills and knowledge of Microsoft Office software preferred • Must reside in Colorado or Montana and be within close proximity to an Intermountain Health facility, preferably under an hour • Ability to work the specified alternating shifts and scheduled weekly hours • Ability to perform required visual, auditory, manual dexterity, computer, communication, and potential driving tasks
