📋 Description

• Work with assigned patients and/or beneficiaries to assess, facilitate, plan, and advocate for individual health needs • Assist with coordinating cost-effective healthcare services • Establish transition plans for post-acute care • Support transitions between levels of care through care coordination • Liaise with patients, consulting and referring physicians, and outside agencies to coordinate appointments, admissions, or surgery • Coordinate written and verbal exchange of information before and after patient visits • Collaborate with healthcare team members on patient care • Complete psychosocial histories and assess family dynamics with patients and families • Review computer records and charts to understand patient history and current circumstances • Assess plans of care to meet patient needs • Educate families, patients, and healthcare team members • Meet organizational goals aligned with Franciscan Values and Service Standards

🎯 Requirements

• Associate's Degree in Nursing/Patient Care — Required • Bachelor's Degree in Nursing/Patient Care — Preferred • Indiana Registered Nurse (RN) state license — Required • Illinois Registered Nurse (RN) state license required within 180 days of hire • Certified Case Manager (CCM) — Preferred • Basic Life Support (BLS) certification from the American Heart Association — Required • 5 years of nursing/patient care experience — Required • 1 year of case management experience — Preferred • Weekend position: Saturday and Sunday, 10-hour days • Up to 20% travel required
