📋 Description

• Deliver patient-centered health coaching integrating medical needs with patient preferences • Triage patient-reported symptoms and side effects according to internal protocols • Escalate symptoms and side effects to Providers when appropriate • Provide diabetes, hypertension, hyperlipidemia, and weight-management care and education via chat, phone, and video • Coordinate care between patients, providers, support teams, and cross-functional clinical team members • Create consults aligned with 9amHealth’s internal protocols for Providers • Document patient care accurately and consistently to meet internal standards • Work closely with Providers to deliver direct care and education to patients with chronic cardiometabolic conditions

🎯 Requirements

• Bachelor of Science in Nursing (BSN) required • Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed • Certified Diabetes Care and Education Specialist (CDCES) or willingness to pursue certification • 3+ years of direct patient care experience in an ambulatory or telehealth setting • Strong empathy, communication, and patient-centered care skills • Adaptable, self-directed, and comfortable working independently • Proficiency with digital tools for care delivery • Flexibility to participate in rotational weekend or holiday coverage • HIPAA-compliant, distraction-free workspace • Ability to work rotational coverage, including 2 weekend shifts per quarter • Must be authorized to work in the United States without visa sponsorship • Active compact RN license in good standing
