📋 Description

• Directly supervise Care Managers, Case Managers, Community Health Workers, and other assigned staff • Conduct regular one-on-one supervision, team meetings, and case consultations • Provide coaching, mentoring, and professional development opportunities • Participate in recruitment, orientation, training, and onboarding of new staff • Monitor staff productivity, performance goals, and quality metrics • Conduct performance evaluations and assist with corrective action plans • Oversee daily ECM/CICM operations and workflow management • Assign and distribute members and caseloads appropriately • Monitor member engagement, outreach activities, and service delivery • Identify operational barriers and implement process improvements • Provide guidance on complex member needs and high-risk cases • Ensure timely completion of assessments, care plans, care coordination activities, and documentation • Review high-risk, escalated, or complex cases and participate in interdisciplinary case conferences • Promote person-centered care planning and whole-person care principles • Collaborate with medical, behavioral health, social service, community-based, and long-term care providers • Conduct chart reviews and documentation audits • Monitor compliance with ECM/CICM requirements, health plan expectations, and DHCS regulations • Track quality indicators and corrective action activities • Assist with internal and external audits and quality improvement initiatives • Monitor productivity and utilization metrics • Support performance improvement plans • Maintain relationships with managed care plans and community partners • Participate in health plan meetings, case conferences, and collaborative initiatives • Perform other duties as assigned

🎯 Requirements

• Bachelor's degree in Social Work, Nursing, Healthcare Administration, Human Services, Public Health, or related field required • Master's degree preferred • Minimum 3 years of experience in case management, care management, social services, population health, or healthcare operations • Minimum 1-2 years of supervisory or team leadership experience • Experience working with Medi-Cal populations and complex care management programs • Experience with ECM, CalAIM, CICM, care coordination, or managed care programs preferred • Ability to stand, sit, talk, hear, and use hands and fingers to operate computer, telephone, and keyboard frequently, up to 20% of the time • Ability to reach, stoop, kneel, and bend up to 15% of the time • Moderate walking up to 15% of the time • Moderate driving up to 50% of the time • Close vision for computer work • Ability to lift up to 25 lbs frequently; push and pull up to 25 lbs
