📋 Description

• Provide day-to-day oversight, coordination, and supervision of ECM and CICM staff and operations • Directly supervise Care Managers, Case Managers, Community Health Workers, and other assigned staff • Conduct one-on-one supervision, team meetings, and case consultations • Provide coaching, mentoring, and professional development opportunities • Participate in recruitment, orientation, training, and onboarding • Monitor staff productivity, performance goals, quality metrics, and utilization metrics • Conduct performance evaluations and assist with corrective action plans • Oversee daily ECM/CICM operations, workflow, member assignment, and caseload distribution • Monitor member engagement, outreach activities, and service delivery • Identify operational barriers and implement process improvements • Provide guidance on complex member needs and high-risk cases • Ensure timely completion of assessments, care plans, care coordination activities, and documentation • Review high-risk, escalated, or complex cases and participate in interdisciplinary case conferences • Promote person-centered care planning and whole-person care principles • Facilitate collaboration with medical, behavioral health, social service, community-based, and long-term care providers • Conduct chart reviews and documentation audits • Monitor compliance with ECM/CICM requirements, health plan expectations, and DHCS regulations • Track quality indicators and corrective action activities • Assist with internal and external audits and quality improvement initiatives • Support implementation of performance improvement plans • Maintain relationships with managed care plans and community partners • Participate in health plan meetings, case conferences, and collaborative initiatives • Perform other duties as assigned

🎯 Requirements

• Bachelor's degree in Social Work, Nursing, Healthcare Administration, Human Services, Public Health, or related field required • Master's degree preferred • Minimum 3 years of experience in case management, care management, social services, population health, or healthcare operations • Minimum 1-2 years of supervisory or team leadership experience • Experience working with Medi-Cal populations and complex care management programs • Experience with ECM, CalAIM, CICM, care coordination, or managed care programs preferred • Ability to supervise Care Managers, Case Managers, Community Health Workers, and other assigned staff • Ability to monitor productivity, performance goals, quality metrics, compliance, and utilization metrics • Ability to conduct chart reviews, documentation audits, performance evaluations, and corrective action plans • Ability to oversee assessments, care plans, care coordination activities, encounters, member contacts, and documentation • Ability to facilitate interdisciplinary collaboration with medical, behavioral health, social service, community-based, and long-term care providers • Ability to support audits, quality improvement initiatives, and performance improvement plans • Frequent computer, telephone, and keyboard use • Ability to drive up to 50% of the time • Ability to lift and push/pull up to 25 lbs
