📋 Description

• Conduct comprehensive assessments of members with chronic medical, behavioral health, and social determinant of health needs • Conduct telephonic member engagement and coordinate care for 50–75% of the day • Compile clinical information and develop individualized care plans with members • Provide evidence-based disease-management education and support • Engage appropriate interdisciplinary care team members • Coordinate care to provide a seamless healthcare experience • Document care-management activities accurately and timely in electronic health records • Collaborate with the interdisciplinary care team to address barriers and maintain stable health conditions • Connect members with health-plan benefits, community resources, and support services • Meet regulatory requirements within specified timelines • Support care-team members through clinical decision-making and guidance • Participate in special projects, process improvement, and mentoring new team members • Participate in team meetings to discuss member status and care strategies

🎯 Requirements

• Active and unrestricted Registered Nurse (RN) licensure in Missouri OR compact licensure in state of residence • Associate’s of Science in Nursing (ASN) degree and relevant experience in a health care-related field (REQUIRED) • Minimum 3+ years of nursing experience • Minimum 2+ years of case management, discharge planning and/or home healthcare coordination experience • Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams • Access to a private, dedicated workspace • Ability to work independently from home and collaborate virtually • Ability to meet performance and productivity metrics, call volume, member engagement, and regulatory requirements • Familiarity with community resources and services • Ability to use healthcare technology tools and electronic health records • Effective verbal and written communication • Excellent customer service and member engagement skills • Experience or willingness to learn care management within Medicare and Medicaid managed care • Bachelor’s of Science in Nursing (BSN) preferred • Experience with Medicare and/or Medicaid members preferred • Experience with social determinants of health, chronic medical conditions, and/or behavioral health preferred • Experience conducting health-related assessments and facilitating care planning preferred • Bilingual skills, especially English-Spanish, preferred
