📋 Description

• Facilitate delivery of appropriate benefits and healthcare information to determine benefits eligibility • Promote wellness activities and support member wellness • Assess, plan, facilitate, coordinate, evaluate, and advocate for options and services addressing comprehensive health needs • Collaborate with members, providers, and community organizations • Address members’ healthcare and social determinant needs • Develop, implement, and support health strategies, tactics, policies, and programs • Coordinate clinical coverage, network management, and related services • Promote quality, cost-effective outcomes • Support successful and timely return to work • Serve Community Care members through a member-centric, team-delivered, community-based care management model

🎯 Requirements

• Active and unrestricted RN Compact licensure • Minimum 3+ years clinical practical experience preferred, including experience with Medicare members; relevant areas include diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care • Minimum 2+ years Case Management, discharge planning and/or home health care coordination experience • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently, including from home • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Efficient computer skills, including navigating multiple systems and keyboarding • Willingness and ability to obtain multistate RN licenses if needed • Certified Case Manager preferred • Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) preferred but not required • Associate degree or nursing diploma required • Bachelor’s degree preferred • Ability to work Monday-Friday, 8:00am-5:00pm EST • 40 hours per week
