âą Conduct proactive outreach and post-hospitalization follow-up to connect members with behavioral health services âą Coordinate appointments and support access to benefits and community resources âą Build trusted member relationships to assess needs and identify appropriate providers and resources âą Address barriers to care and escalate risk concerns to clinical teams when needed âą Partner with clinicians, Provider Success, and internal teams on high-risk member follow-up, provider access, program enrollment, and continuity of care âą Educate members, providers, and facilities on available services and care-management processes âą Resolve access challenges and close care gaps âą Maintain accurate and timely documentation and manage sensitive member information in compliance with HIPAA and organizational standards âą Navigate multiple systems efficiently, communicate effectively, meet performance goals, and contribute to a collaborative, member-focused team environment
âą Bachelor's degree in Social Work, Psychology, or another related healthcare field; OR high school diploma plus 4+ years of experience in a healthcare, managed care, behavioral health, and/or customer service setting âą 2+ years of experience in a healthcare, managed care, behavioral health, and/or customer service setting for the bachelor's-degree pathway âą Strong computer proficiency, including working within multiple systems simultaneously and accurately entering or retrieving data âą Ability to talk and type simultaneously while navigating systems during live member interactions âą Working knowledge of behavioral health or medical terminology âą Strong problem-solving, critical-thinking, and organizational skills âą Ability to prioritize, plan, and multitask while balancing competing priorities âą Ability to pass background checks upon hire and throughout employment, including criminal, employment, education, healthcare sanctions, prohibited parties, and 10-panel drug screening âą Demonstrated ability to establish trust quickly and motivate members âą Ability to manage difficult conversations and sensitive topics âą Skilled in de-escalation, reflective listening, and responding appropriately to member needs âą Ability to problem-solve and arrive at timely solutions âą High-speed internet service with minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds, installed before starting âą Quiet workspace with minimal background noise for calls
