📋 Description

• Work closely with the care management team to provide short-term care coordination and connect members to resources • Provide administrative support to care management team members • Collect data for Health Risk Assessment screenings • Screen members for eligibility, benefits, priority for care management services, and appropriate referrals • Perform transition-of-care duties by contacting PCPs, Medical POAs, specialists, home health agencies, and other providers • Produce and mail routine care management letters and educational materials • Document encounters and contacts; complete monthly reports; maintain comprehensive electronic member records • Motivate members to participate actively in their health and well-being • Provide education and follow-up, arrange PCP visits, and perform care coordination under the Care Manager’s direction • Assist members with accessing medical homes, medical care, social services, and other health-related services • Coordinate resources to address barriers and social determinants of health, close care gaps, and support appropriate access to care • Follow up with members and providers regarding health and social services • Verify appointments and obtain medical records to promote member compliance • Expand knowledge of community resources and services • Facilitate access to housing, food, clothing, prenatal and parenting classes, life-skills education, and mental health services • Facilitate communication and coordinate services between providers and members • Maintain confidentiality and security of member records in accordance with policies and privacy requirements • Build positive working relationships with members, providers, nurse care managers, social workers, agency representatives, and office staff

🎯 Requirements

• High school diploma or equivalent • Possession of a valid State Driver’s License • Two or more years of experience serving the Medicaid population in the Maryland or Washington, D.C. area preferred • Strong interpersonal, communication, and customer service skills • Ability to work effectively with a wide range of constituencies in a diverse community • Knowledge of community agencies and resources • Knowledge of medical terminology • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services • Knowledge of transportation and other barriers to care • Ability to communicate medical information to health care professionals and members of the care management team • Creative and analytical thinking • Solid written and oral skills • Strong problem-solving, organizational, and time management skills • Proficiency with computers and Microsoft Office applications preferred • Understanding of remote work tools and platforms (Microsoft Teams, Zoom, SharePoint, etc.)
