📋 Description

• Engage regularly with patients and track adherence to care plans • Monitor patients' device readings and investigate out-of-bounds values • Maintain detailed records of patient interactions and program details • Stay current on regulations and compliance standards for CCM/RPM services • Ensure patient enrollments comply with legal and ethical standards • Monitor and track care performance against goals • Support patients across multiple medical practices, workflows, teams, and patient populations • Transition between practices according to operational needs while maintaining consistent care quality • Create and track patients' care plans and accompany them throughout their healthcare journey

🎯 Requirements

• National Certification in Medical Assisting • Residence in the US • Strong written and verbal communication skills • Empathy and the ability to connect with patients and their families • Basic knowledge of chronic health conditions and their management • Highly adaptable and comfortable working across different medical practices and teams • Strong interpersonal and communication skills, with the ability to communicate clearly and naturally with colleagues • Reliable, flexible, and comfortable working in a float pool environment • Comfortable using technology for documentation and communication • Strong attention to detail and organizational skills • Ability to work independently and as part of a team • National certification in Medical Assisting from AAMA, NHA, NCCT, NAHP, or similar is required; single-state certifications, diplomas, or completion certificates do not qualify • Attach Medical Assistant certificate • Monday-Friday availability • Bilingual proficiency in English and an additional language is nice-to-have
