📋 Description

• Assist HarmonyCares Primary Care Physicians, Nurse Practitioners and Clinical Teams in delivering primary health care and patient care management in residential settings • Provide compassionate support, attention and assistance to patients, families and team members • Support patient care through clinical partnership with Nurse Practitioners, Physicians and Care Management Teams • Coordinate daily travel itineraries and transport clinicians to scheduled appointments • Prepare patients for examinations, treatments and minor procedures • Secure patient consent forms and accurately document patient care in a timely manner • Coordinate patient care as directed by clinicians, company standards and policies • Maintain patient confidentiality and treat patients with courtesy and respect • Perform venipuncture, EKGs and other procedures requested by clinicians • Label, store and package specimens for laboratory delivery • Identify and close quality gaps • Document and update patient care plans, Social Determinants of Health assessments and other assessments in patient charts or electronic medical records • Maintain compliance with CLIA, OSHA and HIPPAA guidelines • Prepare and maintain treatment supplies and equipment, including supply inventory, medical bag and car • Ensure the company vehicle is clean and follows the required periodic maintenance schedule • Attend required meetings and in-services

🎯 Requirements

• High School graduate or GED equivalent • Graduate of an accredited Medical Assistant, Phlebotomy, EMT, related medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist • Current CPR certification that meets Company guidelines • Knowledge of medical terminology • Basic computer literacy, including EMR, iPad, and GPS experience • Strong interpersonal communication and customer service skills • Must maintain a valid driver’s license and good driving record • Certified Medical Assistant preferred • Six months of experience in patient care, preferably geriatric care • Strong Phlebotomy skills • Desire and ability to obtain MA certification with 180 days of hire • Full range body motion including handling/lifting patients • Ability to drive up to 150 miles daily in urban and/or rural setting • Ability to lift/carry items weighing up to 50 lb • Ability to read and understand road maps • Sit or stand for long periods of time
