📋 Description

• Develop relationships and manage post-acute network performance for Memorial Hermann aligned partners • Collaborate with area post-acute entities and internal and external healthcare teams to optimize outcomes and implement best practices • Improve utilization of the Memorial Hermann aligned Post Acute Care Network and total cost of care for at-risk populations • Hold Preferred Post Acute Facilities/Entities accountable for exceptional patient care, optimized outcomes and alignment with system strategies • Perform care coordination within and between network members • Support timely and well-coordinated care transitions across acute and post-acute levels of care • Perform facility assessments using clinical and operational skills • Review performance metrics and collaboratively create and prioritize action plans • Establish optimal discharge plans and transitions, resolve barriers and support patient progress toward discharge goals • Participate in rounds at network entities, provide feedback and best-practice guidance, and establish accountability • Manage caseload efficiently • Optimize in-network utilization of Memorial Hermann preferred providers • Hold Preferred SNF Facilities accountable for assigning Memorial Hermann patients to Clinically Aligned Physicians • Ensure preferred entities provide required reporting and performance data on time • Notify leadership of service failures or patient-care incidents and trends • Collaborate with entity administration, staff, physicians and practice leadership • Promote professional growth and development and serve as a preceptor, mentor and resource • Ensure safe care and adherence to Memorial Hermann policies, procedures, standards and budgetary specifications • Perform other duties as assigned

🎯 Requirements

• Bachelor’s degree in Business or Healthcare related field required • Active, unrestricted registered clinical license required, such as RN, PT, OT, ST, RT, or Licensed Social Worker • Five (5) years healthcare management experience with a minimum of two (2) years of experience in post-acute • Thorough knowledge of healthcare services, regulations, reimbursement, national trends and operations • Exceptional verbal and written interpersonal and communication skills • Ability to develop and execute marketing/business development strategies • Ability to work collaboratively to improve outcomes • Ability to work independently with minimal guidance • Independent problem identification and resolution • Proficient with Microsoft Office applications including Word, Excel, and PowerPoint • Comfortable presenting in front of clinical and c-suite teams • Comfortable with current at risk contracting and management strategies such as BPCIA and MSSP
