📋 Description

• Accept patient handoffs from an Intake Care Manager and continue care • Develop rapport with patients and create a safe therapeutic environment • Help patients develop insight into themselves, their patterns, and their relationships • Guide patients in managing problems and triggers to increase relapse prevention and reduce risk • Encourage work skills and participation in social, recreational, or therapeutic activities through brief therapeutic interventions, including motivational interviewing, behavioral activation, and problem-solving therapy • Educate patients about medication and medication adherence, and monitor medication-related problems • Report findings to PCPs and Psychiatric Consultants • Communicate treatment recommendations and updates among PCPs, patients, and psychiatrists • Match daily calendars to documentation in Mindoula's electronic medical record (MMR) • Document all billable activity in MMR within 24 hours • Present follow-ups to the psychiatrist during case presentation • Counsel patients about illness, abuse, medication, and community and social resources • Assess risk, develop safety plans, and perform crisis interventions • Monitor, evaluate, and record patient progress using validated monthly assessments • Prepare and maintain required records, reports, paperwork, and diagnostic records while maintaining confidentiality • Communicate clearly with patients, families/caregivers, staff, and the treatment team • Produce high-quality documentation • Relate effectively to patients, families, and involved agencies

🎯 Requirements

• Master's Degree Required in psychology, social work, or another behavioral health discipline from an accredited college or university • Proficient in technology to successfully work remotely, specifically Google Suite (calendar, docs, sheets, slides, etc.) • Strong documentation and data entry experience or abilities • Work experience with Electronic Medical Records
