📋 Description

• Lead a care team of Therapists and Behavioral Health Support Specialists to implement programs, serve members, and deliver outcomes • Act as a player/coach by supporting members directly and building team skills • Create and coordinate member service plans and community resource guides • Guide members in managing problems, triggers, relapse prevention, and social or therapeutic activities • Educate members about illness, prevention, medication, and community resources • Monitor, evaluate, document, and report member progress, critical incidents, and quality-of-care issues • Assess member risk and perform crisis intervention according to safety plans • Ensure members have appropriate care plans and progress toward goals and program graduation • Coordinate caseloads, referrals, and team schedules • Participate in screening, interviewing, and hiring team members • Provide orientation, training, performance reviews, and ongoing coaching • Lead enrollment activities and ensure KPIs and partner outcomes are met • Review documentation and analyze team performance to identify gaps and improvement plans • Collect and synthesize member-outcome data for training, engagement, market understanding, and partner relationships • Lead internal case reviews and participate in external case-management meetings with health plan partners • Maintain program data, reports, statistical records, and other required data collections • Partner with billing to review claims for accuracy and provide additional information

🎯 Requirements

• Master's degree in behavioral health, social work or health care management from an accredited university required • Independent clinical behavioral health license in Washington required; eligible licenses include LMHC, LICSW, LMFT, or Licensed Psychologist • Experience leading, building and implementing programs and delivering outcomes • Experience with Health Coaching or Medical Social Work preferred • Experience with suicide prevention and crisis in adolescent and young adult populations and severe mental illness populations preferred • Experience managing interdisciplinary care teams preferred • Experience ensuring care complies with applicable laws, regulations, policies and procedures • Ability to develop, maintain, and strengthen relationships and partnerships • Track record of identifying needs and independently implementing change • Ability to plan, organize, manage time, prioritize multiple tasks, and follow through • Excellent listening, verbal, and written communication skills • Ability to clearly and accurately relay information to staff, business partners, stakeholders, and clients • Ability to work cooperatively to set goals, resolve problems, and make decisions • Ability to provide case/care management in routine and crisis situations
