📋 Description

• Assess, plan and implement individualized care strategies directed toward the most appropriate and least restrictive level of care • Collaborate with members, families and healthcare providers to develop individualized plans of care • Conduct individual counseling and group therapy with adolescents and adults • Identify and initiate referrals for financial, psychosocial, community and state supportive services • Advocate for members and families to ensure patient needs and choices are represented and supported • Utilize approved clinical criteria to assess and determine appropriate level of care • Document member assessments, care plans and referrals • Achieve set goals and Key Performance Indicators (KPIs) • Learn the StrongWell model and take ownership for outcome-based care • Participate in interdisciplinary team meetings and utilization management rounds • Provide information to assist with safe transitions of care • Promote responsible and ethical stewardship of company resources • Maintain punctuality and attendance during work hours

🎯 Requirements

• Must hold an LCSW, LMFT, LPC, or LMHC license in Florida • Preferred experience with substance abuse populations • Background in maternal and substance abuse care preferred • Experience with adults and adolescents • Familiarity with Medicare and Medicaid procedures • Remote work experience
