📋 Description

• Support members facing substance-exposed living and help them make positive changes • Serve as a role model for individuals in varying phases of recovery • Follow Substance Use Disorder Program guidelines in planning care, assessing client needs, and completing screenings • Refer clients to resources as needed • Coordinate with the Substance Use Disorder Program team, primary care providers, managed care organizations, and community agencies • Identify individual member needs and document interventions based on relationships and assessments • Provide member support in a variety of settings and assist with support groups as needed • Contribute to case management meetings • Complete documentation for member contacts, touches, cancellations, and care plans • Follow chain of command when communicating concerns or recommendations • Achieve set goals and Key Performance Indicators (KPIs) • Represent and promote Mindoula and the Substance Use Disorder program through community participation, professional organizations, and outreach efforts • Participate in conflict resolution and performance improvement processes • Support team members in performing their duties • Promote responsible and ethical stewardship of Mindoula’s resources • Partner with the enrollment team to grow member census and identify eligible members for enrollment • Perform other duties as assigned

🎯 Requirements

• Certified Peer Recovery Specialist • Peer Recovery Specialist or Peer Support Specialist experience • Successfully participated in a recovery program • Caring and compassionate nature • Strong organizational skills • Problem-solving skills • Ability to conduct home visits and travel within a reasonable driving distance from home in Florida • Daytime travel availability of 25%–50% • Excellent punctuality and attendance during work hours • Ability to communicate professionally with members, families, physicians, community agencies, agency leadership, and office staff
