📋 Description

• Primary Care Services: Conduct physical exams, diagnose and treat acute and chronic illnesses, and provide routine primary care to improve patient health outcomes. • Patient Assessments: Take medical histories, perform physical examinations, and order and interpret diagnostic tests. • Chronic Disease Management: Develop and manage treatment plans for chronic conditions such as diabetes, hypertension, and asthma. • Documentation: Maintain accurate and timely patient records using electronic health record (EHR) systems. • Administrative Duties: Including but not limited to assisting with opening/closing the clinic, inventory, patient follow-up, verifying insurance, collecting payment, scheduling appointments, and maintaining a clean environment. • Preventive Care: Administer vaccinations, conduct health screenings, and educate patients on disease prevention and healthy lifestyles. • Collaboration: Work with other healthcare providers, nurses, and medical assistants to deliver comprehensive care. • Community Outreach: Participate in outreach activities to raise awareness about the mobile clinic’s services and engage with the community. • Actively participating in leadership-level planning for the best growth opportunities.

• Values: Value all people and have a desire to be a Servant Leader • Attitude: Embrace our culture and our SERVICE values • Education: Master’s or DNP Family Nurse Practitioner (FNP) with completion of an accredited FNP program, with current Family Nurse Practitioner (ANCC or AANPCB) Board Certification or equivalent certification. • Currently licensed or eligible for California APRN licensure as a Family Nurse Practitioner • Experience: Clinical experience in family and primary care services preferred • Experience with EHR preferred • Performance: Able and willing to provide world-class service to all stakeholders.
