📋 Description

• Provide after-hours triage, clinical assessment, and interventions via phone • Support patients and families with symptom management and end-of-life care needs • Coordinate care by collaborating with physicians, facilities, and internal teams • Dispatch visits and coordinate services with vendors as needed • Deliver clear, compassionate education and support to families • Accurately document all interactions, care plans, and outcomes in the EMR • Review team documentation for accuracy and compliance • Partner with Telecare staff to ensure positive patient outcomes

🎯 Requirements

• Active RN license (no current or pending restrictions) • Hospice experience • Home Care Home Base (HCHB) experience • Strong clinical judgment and critical thinking • Excellent communication and phone-based patient care skills • Ability to multitask in a fast-paced, remote environment • Strong customer service and conflict resolution skills
