📋 Description

• Partner with patients, families, primary care teams, and community organizations to improve access to care • Address social drivers of health • Support chronic disease self-management • Connect patients with medical, behavioral health, and community-based resources • Provide health system navigation and resource coordination • Conduct outreach and education • Reinforce care plans • Provide culturally responsive coaching • Serve as a trusted frontline public health worker • Do not provide clinical assessment, diagnosis, treatment, clinical case management, or services requiring professional licensure

🎯 Requirements

• High School Diploma or GED • Experience in health care, public health, community-based services, human services, care coordination, behavioral health, or social services • Knowledge of local community resources • Experience with Medicaid, rural populations, behavioral health, chronic disease, maternal-child health, or underserved communities • Completion of, or willingness to complete, a Michigan-recognized CHW training program • MiCHWA CHW certification or eligibility is highly preferred • Must live in the East Region of Northern Michigan, near Indian River, Grayling, Gaylord, or Mio areas • Must be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella • Must receive a flu vaccine during the flu season in the year hired and annually thereafter, or receive an approved medical or religious exemption • Bilingual skills preferred where relevant
