📋 Description

• Notify communities/facilities and providers of scheduled visit dates, request consultations, and provide visit schedules • Organize and maintain monthly provider calendars • Create visit templates and reports with scheduling software for monthly flyer distribution • Develop and maintain relationships with key decision makers within communities/facilities • Identify concerns and opportunities for account growth • Maintain updated account information in CRM • Work closely with practitioners to ensure seamless patient care • Collaborate with internal teams to ensure customer satisfaction and address issues • Complete tasks assigned in Planner • Schedule and confirm patient appointments, follow-ups, and referrals • Create and upload schedules for practitioners and facilities • Coordinate with patients, Power of Attorneys, and facilities regarding appointments and concerns • Notify facilities of upcoming visits and request patient sign-up lists and missing documentation using CRM • Report the number of patients seen at each facility • Respond to voicemails in a timely manner • Build facility relationships by phone and email to encourage growth • Monitor insurance logs for new insurance approvals • Monitor outstanding referrals and authorization requests • Monitor the Reschedule queue in eDerm and reschedule patients as needed • Monitor the fax folder to confirm all items are addressed • Organize practitioners' calendars in Outlook with assigned facilities and times • Template facilities in software and run/check reports for scheduling accuracy • Format reports into monthly flyers for distribution • Collaborate with marketing, management, and patient care coordinators to review concerns and growth opportunities

🎯 Requirements

• High School Diploma (or equivalent) • Prior Healthcare Experience: Background in patient care coordination, patient scheduling • Strong understanding of insurance eligibility using Availity and Payor specific platforms • Outstanding Communication: Friendly, clear, and professional phone manners • Organizational Proficiency: Able to handle multiple tasks seamlessly and accurately in a fast-paced environment • Tech Savvy: Comfortable navigating medical software and EHR systems • Compassionate Mindset: A genuine desire to help others and prioritize patient care • High-speed internet access
