• Maintain and complete current and accurate Trauma Registry databases for inpatient and outpatient trauma patients • Abstract patient-care information from pre-hospital, emergency room, hospital course, and final outcome phases • Abstract data from patient medical records, admission logs, and nursing records • Input hospital course and discharge variables into the database • Review and validate registry data for accuracy and completeness • Request chart retrieval from the Medical Records Department • Assist with daily operations of the trauma registry • Assist with statistical reports supporting Trauma Program Operations and the Trauma Performance Improvement Program • Prepare required data for EMS/Trauma Committees in collaboration with the Trauma Specialist • Assist in preparing reports supporting ongoing program planning • Provide data input and reports supporting Process Improvement Programs • Monitor compliance with Trauma Service standards of patient care • Audit the Trauma Registry for compliance with American College of Surgeons standards • Report performance improvement issues to the Trauma Program Manager/Trauma Director • Research identified performance improvement issues • Maintain confidentiality of patient and facility records and information • Organize work to achieve productivity and efficiency during assigned shifts • Perform duties independently with minimal supervision and complete tasks within established time frames • Attend department and facility meetings, classes, and safety programs • Perform other duties as assigned
• High school graduate or equivalent • 1–2 years’ experience abstracting data from medical records, preferably ED/Trauma related • Experience in data collection using ICD-10 clinical manager codes preferred • ICD-10 Course Certification within the last 5 years • Ability to understand medical terminology and identify and abstract medical data from paper and electronic medical records • Ability to work accurately under department timelines with good judgment and minimal supervision • Strong typing and computer skills, including data entry and retrieval techniques • Computer and software troubleshooting skills, including spreadsheets and word processing • Ability to read and interpret medical-record documents and enter complete data into an online computer system • Demonstrated ability to analyze and interpret data • Excellent written and verbal communication/interpersonal skills • Tact and diplomacy when obtaining information from hospital and public sources • ATS Trauma Registry AIS05/08 or AIS15 certification preferred • CAISS Course Certification preferred • CSTR Course Certification preferred • EPIC and TraumaOne experience preferred • Must reside in California • Measles, mumps, varicella, and annual influenza immunizations required as a condition of employment • Access to protected health information requires compliance with HIPAA and patient privacy requirements
