📋 Description

• Conduct 70 to 90 outbound patient calls per day to check in on patients and address health concerns • Handle inbound phone calls efficiently and professionally, triaging and routing according to clinical urgency • Route non-clinical inbound calls to appropriate company departments • Conduct and document monthly Chronic Care Management (CCM) care plan calls • Collaborate with providers to coordinate timely and effective patient care via telephonic communication • Document all patient interactions in the clinical platform in real time during each call • Meet or exceed individual and team metrics for care quality, patient engagement, call handle times, response times, and protocol adherence • Adapt to fluctuating call volumes and changing priorities while maintaining patient care standards • Pivot among CCM adherence calls, patient onboarding calls, and comprehensive chart reviews as needed • Maintain compliance with company policies and applicable regulations • Perform other duties as assigned

🎯 Requirements

• Active MA license required • Strong communication and organizational skills • Genuine curiosity about AI and comfort experimenting with Claude, ChatGPT, and workflow automation platforms • Judgment to determine when AI tools are appropriate for clinical documentation and care coordination • Passion for patient care, empathy, and compassion preferred • Passion for patient teaching • Previous call center, teleservice, or high-volume phone-based work experience preferred • Minimum fiber internet speed of 50 Mbps download / 10 Mbps upload • Hardwired internet connection required • Speed test submission required during the offer process • Private, HIPAA-compliant workspace • Mandatory attendance for the four-week training program • Availability Monday through Friday, 11:00 AM–8:00 PM Eastern Time, with training Monday through Friday, 9:00 AM–6:00 PM Eastern Time
