• Conduct targeted telephonic outreach to patients, families, and healthcare organizations • Educate patients and families about palliative care, VyncaCare services, and program expectations • Complete structured eligibility screenings covering demographics, chronic disease history, and functional decline • Confirm service-area eligibility • Schedule eligible patients for initial visits with nurse practitioners • Coordinate enrollment logistics and smooth handoffs from enrollment to care • Serve as a liaison between patients and the broader care team • Direct calls, texts, and emails to appropriate individuals or departments • Respond to patient questions and concerns in a timely, supportive manner • Maintain confidential and accurate patient documentation and communications in accordance with HIPAA and organizational standards • Coordinate enrollment efforts across targeted regions and track outreach activity • Identify outreach trends and enrollment barriers • Share qualitative and quantitative feedback and creative ideas with the market and business development team • Maintain collaborative relationships with the designated market team and practice leadership • Contribute to program expansion, census growth, and patient access
• Current CMA or CCMA certificate issued by a recognized credentialing body • 2 years of healthcare experience required • 8 months of previous clinical experience preferred • 1 year of telephonic or call center customer service experience • Experience working in a medical practice or healthcare setting • Familiarity with EHR platforms for documenting patient interactions and scheduling appointments • Experience and training in motivational interviewing and patient education • Comfort discussing serious illness, chronic disease, and end-of-life adjacent topics with patients and families • Strong organizational skills and ability to work independently and manage a high-volume outreach workload • Proficiency with Microsoft and Google applications • Access to a reliable internet connection • Strong written and verbal communication skills • Exceptional interpersonal skills, including conflict resolution and de-escalation techniques • Willingness and ability to work Monday through Friday, 8:30am–5pm Eastern Standard Time, with possibility of overtime • Bilingual English/Spanish preferred • Medical terminology certification preferred • Familiarity with palliative care, home-based care models, or chronic disease management preferred • Legally authorized to work in the United States • Must not require employment visa sponsorship
