📋 Description

• Lead the design and execution of program activities, ensuring timely delivery, strategic alignment, organizational goals, and measurable outcomes • Build and maintain relationships with internal teams, leadership, and external partners • Support deployment of digital health tools, including remote patient monitoring and patient-facing technologies such as patient portals • Apply program and project management practices including governance, prioritization, resource alignment, risk management, and quality oversight • Leverage data and analytics for decision-making, continuous improvement, and program or product roadmaps • Serve as the primary point of contact for program communications and provide updates, presentations, and training • Improve adoption, implementation, and optimization of tools, technologies, and processes • Promote a health center-centric culture supporting health care organization transformation • Maintain awareness of industry trends, regulatory requirements, adult learning strategies, OCHIN services, and relevant program knowledge • Communicate with health care organizations regarding status, timelines, data requirements, and deliverables • Guide health care organizations in addressing challenges and developing, implementing, and sustaining action plans to improve rural health care

🎯 Requirements

• Bachelor's degree in healthcare, program/project management, or a related field required; master's degree preferred • Minimum of 3 years of experience supporting health care teams in a similar or relevant role strongly preferred; 4–5 years a plus • Experience with patient portals, telehealth, e-consults, and/or remote patient monitoring technology required • Experience with Epic functionality preferred • Knowledge of healthcare data, reporting, and analysis required • Familiarity with advanced primary care concepts, chronic disease management, clinical quality metrics such as UDS, HEDIS, NCQA, or NQF metrics, and FQHC/safety net clinics preferred • Experience supporting clinicians, care teams, and clinic leaders with implementing practice changes in clinical settings • Knowledge of motivational interviewing, human-centered design, Model for Improvement, Lean, PCMH, or Building Blocks of High Performing Primary Care frameworks • Strong process understanding of the project lifecycle and ability to anticipate and resolve problems independently • Ability to drive execution on multidisciplinary, complex, multi-stakeholder healthcare projects and manage multiple projects simultaneously • Strong analytical, problem-solving, written, verbal, facilitation, coaching, and consulting skills • Strong experience in a telecommuter role, including OneNote, SharePoint, Teams, Zoom, and Smartsheet; JIRA experience is a plus • Ability to travel to urban and rural locations up to 25–50%, including nationwide travel for Go-Live support or training • Ability to work independently and efficiently from a home office with high-speed internet and a distraction-free workplace • Required ability to read, speak, write, and understand English • COVID-19 vaccination required, or approved medical/religious exemption, before hire • Completion of a 50–60-minute competency assessment before the team interview
