📋 Description
• Work in partnership with patients, families, caregivers, and providers to deliver targeted, organized, coordinated, cost-effective, and patient-centered care • Collaborate with interdisciplinary staff and stakeholders across the care continuum • Promote patients’ optimal health through disease prevention and management strategies • Apply evidence-based interventions to populations for cost-efficient and outcome-based care • Address disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates
🎯 Requirements
• Bachelor's Degree (Required) • APRN - Advanced Practice Registered Nurse License - State of Ohio Board of Nursing • BLS - Basic Life Support - American Heart Association • RN - Registered Nurse - Ohio Board of Nursing
