📋 Description

• Treat patients via telehealth visits, including telephonic triage calls, video visit appointments, and email follow-ups • Participate in weekly Clinical Rounds and other continuous-learning modalities • Collaborate with virtual and in-office teammates through daily huddles • Apply specific clinical training to provide exceptional virtual patient care • Provide evidence-based, human-centered primary care to One Medical patients in coordination with primary care providers • Deliver virtual clinical care using advanced technology and a team-based approach

🎯 Requirements

• Completed an accredited FNP program with a national certification • In the past 5 years, practiced as an Advanced Practitioner for at least 2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+) • Currently licensed in GA, with ability to obtain additional state licenses • Ability to work weekday and weekend shifts; every other Saturday and/or Sunday required • Ability to work current shifts ranging from 6am–5pm EST, 7am–6pm EST, 8am–7pm EST, 11am–10pm EST, and 1pm–12am EST • Excellent clinical and communication skills • Clinical proficiency in evidence-based primary care • Ability to effectively use technology to deliver high-quality care • Ability to communicate with and provide care to individuals of all backgrounds • Openness to feedback and reflection • Ability to navigate uncertain situations with patients and colleagues • Readiness to adapt personal and interpersonal behavior to meet patient needs
