📋 Description

• Deliver remote patient care through a virtual technology platform • Independently provide holistic, evidence-based assessment, diagnosis, treatment, health-problem management, health counseling, and disposition planning • Provide pregnancy prevention, STI prevention, safer-sex, contraceptive-care counseling, and medication management • Evaluate primary care, acute, chronically ill, and transitional-care patients aged 18 months and above • Educate patients on health maintenance and respond to patient-care inquiries • Document patient care in the EHR according to MinuteClinic policies and procedures • Coordinate care with internal and external colleagues, including the broader patient-centered medical home • Work within a patient-care team with Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

🎯 Requirements

• Current, unrestricted license in any US state/district • Willingness and ability to obtain additional licensure as needed • Minimum of two years of medically-relevant experience or equivalent • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multi-task • Initiative, problem solving ability, adaptability and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Completion of a Master’s Degree level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Broadband connectivity • Quiet setting with a neutral background for remote visits • Ability to uphold patient privacy per CVSH guidelines • Ability to work 20+ hours per week, including some weekends
