📋 Description

• Monitor and analyze physiological data as supplemental support to healthcare providers’ patient care plans • Liaise between patients and providers to support compliance with remote monitoring care plans • Make proactive outbound calls to provide clinical support and education • Maintain accurate records and document monitoring actions and discussions in a cloud-based dashboard • Ensure clear and consistent communication with healthcare providers and patients • Address incoming calls requiring clinical guidance • Liaise with Sales, Marketing, Customer Care, Development, and other departments to optimize patient and provider experiences • Collaborate on customer procedures, policies, and standards • Assist Clinical & Regulatory Affairs with remote patient monitoring-related tasks • Perform duties consistent with the requirements of the quality management system • Assume additional responsibilities as required in an early-stage high-tech company • Report to the Director of Nursing and work as part of a cross-functional team

🎯 Requirements

• LPN or RN with verified credentials and licensure in Puerto Rico • LPN or RN credentials and licensure in any other state is an asset but not required • Located in the USA • Legal authorization to work in the United States • Fluent in both English and Spanish, written and spoken • Must be considered “qualified clinical staff” by the CPT codebook • Minimum 3 years of licensed clinical staff professional service with patient management experience, remote or in person • Previous experience in diabetic care, wound care, chronic disease management, or remote patient monitoring programs is a strong asset • Experience supporting Veterans within the VA/DOD healthcare sector is an asset • Ability to analyze data and formulate clear clinical feedback • Proven record of professional and effective customer communication skills • Ability to establish rapport, build relationships, and diffuse conflict situations effectively • Familiarity with technological troubleshooting and multiple smartphone platforms • Proficiency with Microsoft Office suite • Familiarity with quality management systems, ISO 13485, and HIPAA/PIPEDA standards is an asset • Excellent attention to detail, decision-making, problem-solving, and critical-thinking skills • Self-motivation and ability to deliver under pressure • Ability to thrive in a rapidly growing, fast-paced, high-tech start-up environment • Excellent written and verbal communication skills
