📋 Description

• Support and coordinate high-quality, safe, guideline-concordant virtual care for low-acuity, non-emergent medical conditions through video, phone, and asynchronous messaging • Conduct patient intake and collect health needs and goals • Evaluate health status and care needs • Review medical documentation, referral forms, records, and test results; incorporate information into care plans and share it with appropriate providers • Guide patients toward appropriate therapies offered by Ovatient and its partners • Document concerns, conflicting information, and other issues identified during evaluations • Promote continuity of care and reduce fragmentation, duplication, and treatment gaps • Complete technology checks and help patients navigate care technologies • Work closely with Ovatient providers, the Behavioral Health Team, registered nurses, and interdisciplinary healthcare professionals • Monitor and manage referral queues, process behavioral health referrals, and facilitate referrals to specialized services or community resources • Monitor patient messages, respond timely, address concerns, provide guidance, and escalate issues as necessary • Onboard patients to treatment programs, self-guided resources, content libraries, and secure asynchronous messaging • Provide empathetic, patient-centered care across diverse populations • Maintain accurate, current electronic medical records and protect data security and confidentiality • Participate in continuing education, quality improvement initiatives, and clinical team meetings • Stay current with virtual care advancements, clinical best practices, and virtual care technologies

🎯 Requirements

• Completed an accredited medical assistant program, nursing assistant program, or an LPN with compact license • Current national certification in medical assisting from an organization such as AAMA or NHA • Minimum 5 years of clinical experience in primary care or urgent care • Intermediate to advanced computer skills, including navigating multiple systems on one monitor and learning software programs and applications quickly • Strong proficiency in telemedicine and virtual care technologies • Experience and proficiency with Electronic Medical Records (EMR) software • Exceptional communication, interpersonal, and problem-solving skills • Effective telephonic and virtual communication skills with demonstrated ability to provide excellent customer service • Strong commitment to patient care, quality, and clinical excellence • Comfortable working independently in a fast-paced, dynamic environment • Availability to work between 7am–4pm EST • High-speed, reliable broadband internet connection • Dedicated private workspace isolated from interruptions to protect confidential information and comply with HIPAA privacy laws
