📋 Description

• Provide high-quality, prompt assistance with triaging and coordinating transfers from other facilities • Monitor and respond to transfer center triage needs • Complete the direct referral acceptance process • Review patients’ clinical needs using technology • Communicate prompt acceptance or deflection decisions • Collaborate with referral sources, physicians, nurses, and other stakeholders • Report to and remain accountable to the Director of Assessment and Referral for Voyages

🎯 Requirements

• Valid LVN license in the State of Texas • Previous experience in behavioral health and substance use field preferred • Experience working with case management, hospitals, and admissions preferred • Excellent customer service skills • Excellent verbal and written communication skills • Organizational and multitasking skills • Ability to work varied shifts supporting a 24/7 department, including day and overnight coverage • Shift times may vary across different US time zones
