📋 Description

• Lead the design, implementation, and oversight of housing programs supporting Medicaid members facing homelessness • Act as a subject matter expert and advise internal teams on housing-related challenges • Collaborate with interdisciplinary teams to provide holistic and integrated service delivery • Build and maintain partnerships with shelters, housing providers, property developers, and local authorities • Customize housing solutions to meet members' unique needs, accessibility, and sustainability requirements • Monitor and evaluate program effectiveness and make data-driven adjustments • Train and support case management teams on housing resources and transition strategies • Liaise with community organizations and stakeholders to address housing issues and identify collaboration opportunities

🎯 Requirements

• Bachelor's degree in Social Work, Human Services, Public Health, Urban Planning, or a related field; equivalent experience will be considered • Minimum of 2 years of experience in case management, housing coordination, or a similar role • Knowledge of Medicaid regulations and eligibility requirements • Experience working with diverse populations, including individuals with disabilities and low-income families • Excellent communication, organizational, and interpersonal skills • Ability to work independently and as part of a team in a dynamic environment • Valid driver's license and reliable transportation for travel to client meetings and housing sites • Preferred: Licensed Social Worker (LSW) certification • Preferred: Familiarity with local housing resources, services, and community-based organizations • Preferred: Bilingual or multilingual abilities • Preferred: Experience developing or managing affordable housing projects • Preferred: Familiarity with Medicaid programs and Social Determinants of Health (SDoH)
