📋 Description

• Initiate phone contact with discharged patients from specified units • Conduct post-discharge outreach calls within the CMS-required timeframe • Review discharge summaries, medication lists, and follow-up instructions before patient contact • Provide medication reconciliation and coordination support • Assess patient understanding of discharge plans, symptom management, and medication adherence • Record patient responses, advice provided, variations, outreach attempts, interactions, and outcomes in the EHR • Identify potential complications and escalate concerns to providers or the care team • Schedule primary care appointments and coordinate with front desk and clinical teams • Reinforce discharge education, including when to contact a provider or seek emergency care • Support readmission prevention by ensuring follow-up and addressing barriers to care • Review and address care gaps such as hypertension control, diabetes monitoring, preventive screenings, and wellness visits • Educate patients about overdue screenings or visits and assist with scheduling • Collaborate with providers, care coordinators, and case management teams to ensure continuity of care • Participate in team discussions and quality improvement initiatives • Maintain compliance with documentation and billing requirements • Provide hypertension-focused education and intervene on elevated blood pressure readings or medication concerns • Collect patient data, complete forms, identify needs and priorities, review diagnostic results, and perform advanced nursing observations • Develop follow-up care plans using the nursing process in collaboration with patients, families, and other disciplines • Provide telephonic care advice and follow-up instructions according to patient care standards • Perform other duties as assigned

🎯 Requirements

• Associate degree in Nursing required; Bachelor's degree in Nursing preferred • One (1) year experience as a registered nurse • Current RN compact/multistate license recognized by the NCSBN Compact State, or license to practice as an RN in the state where the team member is working • In lieu of an associate degree in nursing, an RN diploma or certificate with the required current RN license is accepted • Knowledge of and compliance with Prisma Health's purpose • Ability to conduct post-discharge outreach calls, medication reconciliation, patient assessment, telephonic care advice, documentation, and follow-up coordination • Understanding of age-related patient needs, patient rights, confidentiality, ethical issues, and accepted nursing practice standards
